Our Care Team Pharmacists provide direct care of complex patients with chronic cardiometabolic conditions. Additionally, they are responsible for managing, implementing, and optimizing our clinical and patient education experience. In the spirit of transparency, we anticipate hiring for this position in Q4.
What you’ll do
Provide direct clinical care for our patients
Create thorough consultations for our 9amHealth physicians
Adjust medications for cardiometabolic conditions under a collaborative drug treatment agreement (CDTA)
Comply with all regulatory statutes related to the practice
Reinforce quality measures and standards within the clinical department
Create and implement therapeutic interchange programs
Support clinical team members to provide group-wide feedback for product updates and new feature implementations
What you’ll need
Minimum Qualifications
Valid Pharmacist license
Experience working directly with patients and HCPs
Thrives in a fast-paced, innovative environment
Empathetic and an expert in patient-centered care
A quiet, distraction free dedicated work space
Preferred Qualifications
Experience with telehealth
Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM) or eligible for certification
