Job Description
As a Medical Assistant - Health Information Coordinator you will be supported by clinical teams to strengthen the connection between patients and their primary care physicians/medical practice staff, as well as to enhance patients’ health and well-being. You will be responsible for managing, organizing, and maintaining patient health records and information. This fully remote position involves obtaining medical records, uploading documents into electronic health records (EMR) systems, and assisting with patient scheduling. The Coordinator plays a critical role in ensuring that patient information is accurate, secure, and readily accessible to support optimal patient care and operational efficiency.
What You’ll Be Doing
- Working in conjunction with virtually embedded Healthguides and RN Case Managers (CM) to coordinate care and provide comprehensive support.
- Obtain and review patient medical records from various sources
- Ensure accuracy and completeness of records before uploading into the EHR system.
- Contact patients to assist with scheduling appointments, including follow-up visits, diagnostic tests, and specialty consultations.
- Providing coverage for Healthguides as needed.
- Functioning independently while being a highly collaborative team member as an extension of medical practice.
- Assisting patients in achieving better health outcomes by addressing their questions and concerns, connecting them with the medical practice, partnering in implementing high-impact care plans and closing care gaps, escalating medical issues, and navigating them to the right resources, including assisting with referrals to specialty care and community-based organizations (CBOs).
- Utilizing effective communication modalities (calls, texts, portals, emails) to engage high-risk patients.
- Appropriately escalating medical issues to the Health Guide Registered Nurse, Primary Care Physician, or Guidehealth leadership.
- Accurately documenting patients’ medical history and issues in the electronic health record (EHR).
- Understanding the referral and prior authorization process.
- Continuously building a trusting, professional relationship with assigned medical practices and within the Guidehealth organization.
- Implementing and meeting assigned Key Performance Indicators to produce positive patient health outcomes while decreasing medical spend.
- Adhering to all security policies and procedures to protect PHI (Protected Health Information) and Guidehealth’s Intellectual Properties.
Qualifications
What You’ll Need to Have
- Certified or Registered Medical Assistant.
- A passion for patient coordination and serving individuals with complex medical and social needs.
- Empathy and proficiency in motivational interviewing, critical listening skills, and behavioral change management to enhance patient engagement.
- Exceptional conversational skills and the ability to precisely document patient interviews in real-time using software.
- Experience and understanding of medical terminology and the function of a primary care physician’s practice.
- Knowledge of social determinants of health.
- Demonstrated use and comfort with documentation within electronic health records (EHRs), care management analytics databases, and various software programs.
- Ability to balance competing priorities while remaining detail-oriented and focused.
- Familiarity with a variety of electronic health records, such as EPIC, Athena, and Cerner.
- Exceptional written, visual, and verbal communication skills.
- Ability to track the progress of multiple tasks while ensuring their completion.
- Capability to work independently as part of a patient’s overall care team under the supervision of licensed clinical professionals.
- Demonstrated ability to work successfully in a remote work environment.
Additional Information
The base pay range for this role is between $20 to $23 per hour paid bi-weekly

COMPENSATION:
The listed compensation range listed is paid bi-weekly per our standard payroll practices. Final base pay decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
