Included Health is looking for a Women’s Health Nurse Practitioner with expertise in infertility and obstetrics, who is passionate about holistic and patient-centered care to support members through their healthcare journey and ensuring needs are met with industry-leading interventions.
The nurse practitioner will guide members through complex medical situations through our maternity care management program, intended for supplemental support to a patient’s local prenatal care. The nurse practitioner will partner with a multidisciplinary clinical team that includes a nurse care manager, dietician, pharmacist, social worker and other healthcare professionals to deliver integrated remote care in an innovative way. The nurse practitioner will support members with education, advocacy, and care management through family building/fertility journeys and prenatal/postpartum care, ensuring they receive comprehensive care that results in positive health outcomes for both the individual and their families.
\n- Deliver coordinated and compassionate virtual Care Management by telephone and/or video that improves members’ health outcomes.
- Generate impactful care plans together with members, nurse care managers and our multidisciplinary care team, to help members achieve the desired goals.
- Partner with the members’ local providers to ensure coordinated care. Coordinate necessary resources that holistically address members’ problems, whether clinical or social.
- Provide clinical depth to a member’s care and help to identify opportunities to improve a member’s health and health outcomes
- Provide thought leadership for maternity care management program.
- Other duties as assigned to meet programmatic needs.
- Active FNP or WHNP medical license and Board Certification.
- At least 3-5 years of post-graduate experience in maternity care.
- Be comfortable guiding maternity based care for a variety of members.
- Obstetrics background and experience with management of infertility and menopause is highly valued.
- Reside in RN compact state.
- Experience with telehealth is highly valued.
- Have an empathetic approach. We work with members and their families who are going through challenging times.
- Ability to coordinate all available resources to holistically support members, with sensitivity to cultural and socioeconomic factors impacting their care.
- Effectively collaborate with multidisciplinary teams, including clinical and non-clinical members, to deliver a seamless, high-quality care experience for members.
- Exceptional verbal and written communication skills, with the ability to gather detailed clinical histories, respond to member questions at an accessible level, and succinctly convey findings.
- Strictly follow security and HIPAA regulations to protect our members’ medical information.
- Ability to work efficiently. We are a fast growing company and we are busy. Our team is expected to meet volume goals without sacrificing quality. Good judgment for balancing priorities is highly valued.
- Be flexible and comfortable with working in a rapidly-changing environment.
- Strong competence and ability to use multiple computer/medical record systems.
- Prompt and regular attendance at assigned work location.
- Capability to remain seated in a stationary position for prolonged periods.
- Eye-hand coordination and manual dexterity to operate keyboard, computer and other office-related equipment.
- No heavy lifting is expected, though occasional exertion of about 20 lbs of force (e.g., lifting a computer \/ laptop) may be required.
- Capability to work with leadership, employees, and members in an appropriate manner.
The United States new hire base salary target ranges for this full-time position are:
Zone A: $108,890.00 - $141,560.00 + equity + benefits
Zone B: $125,224.00 - $162,790.00 + equity + benefits
Zone C: $136,113.00 - $176,950.00 + equity + benefits
Zone D: $141,557.00 - $184,030.00 + equity + benefits

Starting base salary for the successful candidate will depend on several job-related factors, unique to each candidate, which may include, but not limited to, education; training; skill set; years and depth of experience; certifications and licensure; business needs; internal peer equity; organizational considerations; and alignment with geographic and market data. Compensation structures and ranges are tailored to each zone's unique market conditions to ensure that all employees receive fair and competitive compensation based on their roles and locations.
