Description

  • Proactively reach out to Support Coordinators to promote our services and establish collaborative partnerships.
  • Educate Support Coordinators about our capabilities and how we can effectively support NDIS participants.
  • Serve as a primary point of contact for Support Coordinators, addressing inquiries and providing timely updates.
  • Build and nurture long-term relationships with Support Coordinators to encourage participant referrals.
  • Collaborate closely with Support Coordinators to understand participant care plans and requirements.
  • Facilitate seamless communication between Support Coordinators and internal teams to align on service delivery.
  • Coordinate and manage the rostering of staff to execute participant care plans efficiently.
  • Allocate tasks to team members, ensuring alignment with participant needs and organizational priorities.
  • Monitor task completion and address any issues or delays to maintain high service standards.
  • Maintain accurate records of Support Coordinator interactions, referrals, and participant data.
  • Provide regular reports on outreach activities, referrals, and service delivery outcomes.
  • Identify opportunities for process improvements in rostering and service coordination.



Hard Requirements:

  • Proven experience in outreach, relationship management, or a similar role, preferably within the NDIS sector.
  • Strong understanding of NDIS operations, Support Coordinator roles, and participant care plans.
  • Excellent communication and interpersonal skills.
  • Experience with rostering, task allocation, and workforce coordination.
  • Familiarity with NDIS compliance and reporting requirements.
  • Experience working with a provider organization in the NDIS ecosystem.



Schedule:

  • 20 hours/week
  • Monday - Friday | 9AM-1PM Brisbane Time
